Patient tools

Turn what you notice into something you can use.

These tools help you organize meaningful changes, clarify what matters to you, and prepare for conversations with your healthcare team. They do not diagnose lupus activity, recommend treatment changes, or replace clinical assessment.

Privacy in the current version

NA7N is currently a static website. These tools are presented as prompts and templates; this version does not provide patient accounts or longitudinal health-data storage.

Tool 1 · Meaningful change record

Something changed—capture the useful part.

You do not need to record every sensation. Use this when something is new, persistent, clearly different, or important to your daily life.

What changed?
____________
When did it start?
____________
What pattern do I notice?
____________
What happens with it?
____________
How is it affecting my life?
____________
What changed around the same time?
____________
Tool 2 · Quality-of-life priority

Choose one thing you want your care to help protect or improve.

What matters to me?
____________
What is making it difficult?
____________
What would “better” look like?
____________
What help or decision do I need?
____________

Your goal can be practical: enough energy for family time, sleeping well enough to concentrate, continuing work or study, moving with less difficulty, or planning safely for pregnancy.

Tool 3 · One-page visit brief

Bring the essentials into the appointment.

My main health change:
____________
Its impact on my life:
____________
My priority:
____________
The decision I need help with:
____________
My three questions:
1. ___ 2. ___ 3. ___
My updated medicine list:
ready / needs updating
Tool 4 · Understand my medicines

Turn a medicine list into a plan you understand.

Medicine / supplement
____________
Why do I use it?
____________
How was I told to use it?
____________
What monitoring was I asked to complete?
____________
What safety instructions was I given?
____________
My question about it
____________

Do not use this template to decide doses or make treatment changes. If an instruction is unclear, confirm it with your healthcare team or pharmacist.

Tool 5 · Tests and follow-up

Know why a test matters before focusing on the number.

Test / monitoring item
____________
Date
____________
Why was it requested?
____________
What did my clinician explain?
____________
What happens next?
____________
My question
____________

A single result may not explain your health by itself. Review results and trends in their clinical context rather than using this template for self-diagnosis.

Tool 6 · Review what happened

Did the plan help what matters to you?

What were we trying to improve or protect?
____________
What changed since then?
____________
What is better, worse or unchanged in daily life?
____________
What do I want to review next?
____________

This closes the self-management loop: Notice → Prioritize → Discuss → Act on the agreed plan → Review.

Measurement boundary

These NA7N templates are not validated clinical questionnaires.

They are organizational and communication tools. NA7N will distinguish them from validated patient-reported outcome measures and will not present an unofficial translation, restricted instrument or informal score as a validated measure of lupus activity.

Safety

Do not complete a form when you need medical assessment.

Severe or rapidly worsening symptoms require medical assessment rather than waiting to finish a tool or collect more information. Follow any individualized urgent-care instructions already provided by your healthcare team.
